Your order form is automatically sent to your email after placing your order. Please check your inbox, spam, or promotions folder.
Log in to your account and go to https://goodlabs.com/orders
a. Select your order
b. Click the 3 dots in the upper right corner of your order
c. Click “View Order Form”
Note: You can show the order form directly from your phone at the lab, or download and print it if preferred.\
